Ethics Committee UK FTVS

Research that includes human subjects must be approved by UK FTVS Ethics Committee before starting collection of data. Please note that UK FTVS Ethics Committee approves of research on human subjects only, and in the Czech Republic only. It does not consider research on animals, nor research that takes place abroad. Such approvals should be sought from other relevant ethics committees. If in doubt, consult your supervisor.


You should hand in the application form 3 months before you plan to collect data, and in case of complex research, even earlier. To apply you must complete two documents:


1) Application form – Here

Please fill in this form in line with your research and send the application form back to the e-mail address well before the research starts (if you are a bachelor physiotherapy student, send it within 3 days after receiving a patient). Please remove the red sentences from the document - they are there just to help you to fill in the document.


2) A document with Informed Consent

Please note that the document with Informed Consent must be written in a language that your patient/subject can understand. It is supposed that your patients/subjects will understand Czech language - therefore the document with Informed Consent is in Czech. If not, contact the UK FTVS Ethics Committee.


Here – for bachelor physiotherapy students

Fill in the document. Then it needs to be presented to the patient who is supposed to consider it and voluntarily sign it. It is the researcher (you) who must present the document to the patient, OR, if the researcher is in a dependent relationship with the patient, this document must be presented to the patient (and also signed) by another person who is informed about the project, but is not in a dependent relationship with the patient (e.g. a co-researcher) - (see the note no. 3 in the explanatory notes in the attached document).


Here – for everybody else

Fill in the document. Then send it to the UK FTVS Ethics Committee for approval as an attachment to the application form. After approval, it needs to be presented to the patient who is supposed to consider it and voluntarily sign it, after receiving all relevant information. It is the researcher (you) who must present the document to the patient, OR, if the researcher is in a dependent relationship with the patient, this document must be presented to the patient (and also signed) by another person who is informed about the project, but is not in a dependent relationship with the patient (e.g. a co-researcher).


When your Application is approved (you will receive an e-mail from the UK FTVS Ethics Committee), bring the signed original document with Informed Consent to the administrator of UK FTVS Ethics Committee, Ms. Koprivova, office E305, who will keep it, and she will give you the original signed Application Form.


The original signed Application for Approval by UK FTVS Ethics Committee and the document with Informed Consent (just the text, without the signatures and name of the patient/subject) is for the researcher. If the research is a part of a bachelor, master or doctoral thesis, it must be included as a supplement.
