On this page, you will find forms for the most common administrative requests related to your studies at the Faculty. Please select the appropriate form, complete all required fields, and submit it according to the instructions provided below.
Before submitting your application, make sure that:
all required fields are completed,
your student number is included,
the form is dated and signed, and
any required supporting documents are attached.
Completed forms may be submitted to the International Student Office (room H063) or the Registry Office (room H050), unless stated otherwise on the form.
